Application Records & Attestations

  • Active Arizona TPT license number and property location code when applicable

Confirm the active license number and the Flagstaff property location before submission.

  • STR property address and owner contact information

    Required

Use the legal property address and current owner contact details.

  • Owner-designee information, when applicable

Identify each person authorized to act for the owner and covered by the application attestation.

  • 24-hour emergency point-of-contact details

    Required

Use a public contact who can respond by phone, text, or on-site within the required time.

  • Neighbor-notification addresses and planned delivery method

    Required

List the required recipient addresses and how written notice will be delivered.

  • Proof of lawful presence unless exempt under Arizona law

    Required

Provide the lawful-presence evidence requested by the application unless a statutory exemption applies.

  • Applicant acknowledgements and required background attestations

    Required

The owner reviews and signs the legal-compliance and qualifying-background statements.

  • At least $500,000 in liability coverage or qualifying marketplace coverage

    Required

Maintain the required policy or verify equal-or-greater marketplace coverage before offering the property.

The applicant must review the attestations

Chapter 3-12 requires statements about legal compliance and the owner and designee’s qualifying background. Treat these as owner-reviewed legal attestations, not routine contact fields.

Checks That Repeat

  • Before every stay

Complete or verify the booking-guest NSOPW check no later than 24 hours before arrival and retain evidence for 12 months.

  • When fire conditions change

At Stage 2 or higher, secure qualifying outdoor flame devices and keep egress clear of snow, ice, and other obstructions.

  • When the emergency contact changes

Update the license account and send new written neighbor notice within 10 days.

  • Every year

Renew the license, pay $250, refresh application information, and service the extinguisher at least annually.

Common Failure Points

  • TPT address does not match

Update the Arizona license location before resubmitting the City application.

  • Neighbor list is incomplete

Recheck diagonal and same-floor recipients rather than using only the immediately adjacent parcels.

  • Payment was made but no license arrived

Allow the published 2–3 business days after confirmed payment, then use the City contact information on the program page.

  • Marketplace screening is assumed

Verify that the marketplace actually performs the required check; otherwise the owner remains responsible.

Your Flagstaff STR Action Plan

Separate one-time licensing work from requirements that repeat for every stay, every renewal, or changing fire conditions.

  • Build the application packet

Reconcile the property, TPT, owner, designee, emergency contact, notice-plan, and attestation fields before submission.

  • Create a pre-rental gate

Do not publish or host until notice, posting, insurance, safety, and license-number steps are complete.

  • Create recurring records

Track every-stay screening, fire status, contact changes, extinguisher service, and annual renewal separately.

Turn the Flagstaff application into a working checklist
